Hi. Does this header match the Crawling Hedorah? On the other side of the header, the foiler sticker states 2004 as the year. Thanks!

That is the crawling Hedorah by CCP. I don't know what particular release this is, and I think there are at least a couple of editions of the figure, with some special releases and collaborations, but I am sure you can track down more specifics about it or someone here might know off the top of their heads. He is from the AMC/Artistic Monsters Collection. And yes, that is the header for it from what I can tell (ヘドラ = Hedorah, 上陸期 = landing stage).
