You can do a shuttle van like Blue Van, I think it's like 15 or 20 bucks, and there's a bus that runs Grand Central and Penn Station that's cheaper than a cab. But there is no Subway from LaGuardia. I'm not sure if LaGuardia has flat rates to Manhattan. I fly out and in of there all the time, and have never been given a flat rate. But I'm on the UES so it's a very quick trip for me. Avoid the black cars, and if you pass through a toll, they charge it on at the end of the trip. Don't think they're just adding $5 on to hustle you.
