I know!!! In all seriousness though, it's always a crap shoot with the japanese sizes. I asked a few friends I have in Japan before ordering.

I normally wear a large for traditional western sized tees but for the HxS shirt I got an XXL and it fits like somewhere between an L and XL.

I said Large (I usually wear US Medium because I like the slim fit look) and when I reviewed the size listing in centimeters it was the same size of tee's I wear here in Medium. However when after sending payment and replying to the email saying yes that is the correct size, I second guessed myself and thought wait I wonder if he'll think I meant US Large or Japan Large? So I wrote back a second time and said "I mean Japan Large.....US Medium"

Sometimes I over think things so much I want to jump out of a window ......

Like when I ordered 10 devilman shirts in a size large and they won't even fit over my giant head.

So I got an xxl in the brutal monster shirt to be safe.

LOL. Wait I have a big fat head, hmmmm. Eh whatever, as long as it's not a japanese medium or small I should be ok. I never throw t-shirts in the dryer unless they're too big to shrink them.

I usually wear large and ended up picking up a xl at sdcc because that's what was recommended. Fits perfectly actually. I just hope it doesnt shrink when I wash it.

I'm normally fine in both US and Japanese M's but it was advised by HS that you size up since these will shrink in the wash for sure. So I requested an L (and hopefully this will be fulfilled considering I am still email-less).
